When the break suddenly failed, a train with 1,200 passengers on board was slowly climbing the hill at Igandu, Tanzania. 
The 22-car train rolled down the slope into another train at the bottom of the hill, waiting to make the ascent. 
The worst train accident in African history had caused 281 deaths and hundreds injured.
